Usually it's one of four things: mechanics that leak energy (the most common and most fixable), not enough real strength to power the throw, a tired, overused arm, or simply physical development timing. The good news is the biggest one is the most fixable: it's common for a young pitcher to add several miles per hour just by cleaning up a delivery that's been giving speed away.
It's one of the most frustrating things for a pitching parent to watch. Your son is putting in the work, throwing all the time, clearly wants it, and the radar gun just won't move. The natural instinct is to tell him to throw more, or throw harder. That instinct is almost always wrong, and often it makes the plateau worse.
Velocity that's stuck is usually stuck for a reason, and once you know the real reasons, you can stop guessing. There are four that cover almost every case.

1. His mechanics are leaking energy
This is the big one, and the encouraging one, because it's the most fixable. Velocity is built from the ground up and passed up the body like a chain. When a link in that chain leaks, the ball never gets the speed the body created. The usual culprits: the hips flying open too early, no separation between the hips and shoulders, a soft front leg that buckles instead of bracing, and an all-arm delivery that leaves the legs out of it. A young pitcher can often add several miles per hour in a single off-season not from getting bigger, but from finally stopping fighting his own mechanics. 2. The engine isn't strong enough yet
Velocity comes from everything but the arm. The legs and core are what generate the force the arm delivers, so if that engine is underbuilt, there's a hard ceiling on how hard he can throw no matter how clean the mechanics. 3. He's throwing on a tired arm
A fatigued arm can't throw hard, full stop. If he's pitching year-round with no real down time, or throwing max-effort every day, his velocity will flatten or even drop, and the injury risk climbs with it. 4. He's just not there yet physically
Sometimes the honest answer is time. For younger pitchers especially, velocity tracks closely with size, strength, and physical maturity, and a bigger, early-developing kid will simply throw harder than a smaller, late-developing one the same age. That gap almost always closes. Velocity comes in jumps, often around growth spurts, with plateaus in between. A plateau isn't a dead end, it's usually the flat part before the next jump. Measure him against himself, not the kid throwing harder on the other team.
